Sepulchral Sexton

Animated Altar-Guard

Six skeletal figures fused by iron bands and ghostly sinew stand arranged like a dark chorus around an altar, their empty eye-sockets filled with a cold inner light. When the Sexton moves, the bones do not clatter independently — they shift as one mechanism, limbs interlocking and separating with an eerie, practised choreography. It is terrifying not merely because of the bones, but because each strike is coordinated, as though six corpses share a single will to keep something beneath the altar undisturbed.

Actions

Multiattack. The Sepulchral Sexton makes six Bone Spear attacks, using its coordinated limbs to strike different targets or concentrate on one foe.

Bone Spear. Melee Weapon Attack: +6 to hit, reach 10 ft., one target. Hit: 9 (1d6+6) piercing damage.

Wardbreaker (Recharge 5-6). The Sexton slams the central yoke into the ground and unleashes a pulse of binding necrosis in a 20-foot radius centered on itself. Each creature of the Sexton's choice in the area must succeed on a DC 13 Constitution saving throw or be restrained by ghostly chains until the end of the Sexton's next turn. A restrained creature can use an action to make a Strength check against DC 13, ending the effect on a success. Undead are immune to this effect.

Lore

The Sepulchral Sexton is an old warding construct born where necromantic rites met communal devotion. Long ago, a conclave of grave-priests bound six sacrificial cultists—either volunteers or victims—into a single sentinel to protect altars of taboo power. The ritual fused their skeletons with iron filigree and a sigil of binding; the result is not merely undead but a ritual device that enforces silence and custody. Townsfolk whisper that a Sexton will not abandon its post even when the altar it guards has crumbled to dust. Sextons are most often found in subterranean shrines, hidden crypts beneath ruined temples, or the inner sanctums of secluded monasteries. The Ossuary Conclave, a secretive order of archivists and corpse-keepers, maintains records of several Sextons and reveres them as sacred wardens. Scholar Jorren Valem of the University of Kest proved that striking the iron filigree in a specific harmonic sequence can briefly unbind a Sexton's coordination, but Valem's notes are kept under glass after his assistant disappeared while attempting the sequence. Raiders and tomb-hunters prize the secret phrases and plaques that can pacify or redirect a Sexton, but the most common fate for intruders is to face those six coordinated blades.

Appearance

Imagine six humanoid skeletons scaled slightly larger than average, their bones yellowed and pitted with age. Each skeleton is bound by blackened iron bands at the shoulders, hips, and along the ribs; the bands interconnect with thin, rune-etched chains that loop in and out of their ribcages so the bones can articulate in unison. Instead of a single skull jerking with separate necks, their collarbones and upper spines interlock on a central yoke that allows the heads to tilt independently but forces the torso movement to be communal. Threads of pale, translucent sinew — like frozen cobwebs — glow faintly along the chains when the Sexton exerts necrotic energy. The altar-reach limbs end in elongated, blade-like forearm bones and finger bones tapered into spear points; these are polished smooth from ritual handling. Scale reference: the Sexton stands roughly as tall as a human through its central shoulder yoke, but when it spreads its six arms it occupies space closer to a large doorway. Textures: bone grain, rusted iron, faint necrotic filigree, and ghost-veined sinew.
