Quincemort Host
Altarborne Cadre
When a desecrated altar shudders open, five gaunt, blighted forms pour out as if fused at the hip, moving with the hunger of a single mind. The Quincemort Host smells of stale iron and a sweetness like rot; its voices are a chorus of ragged breaths and a faint, pleading chant that unnerves even the stoutest defenders. In battle the five figures fan and converge like a predatory pack, their claws ripping at armor while their collective howl leaves survivors stunned and choking on the memory of death.

Multiattack. The Quincemort Host makes three Claw attacks.
Claw. Melee Weapon Attack: +6 to hit, reach 5 ft., one target. Hit: 10 (2d6+3) slashing damage. If the target is a creature other than an undead, it must succeed on a DC 15 Constitution saving throw or become paralyzed until the end of its next turn as the Host's fetid touch congeals through muscle and breath (the target can repeat the saving throw at the end of each of its turns).
Putrescent Howl (Recharge 5–6). The Host emits a keening chorus. Each creature of the Host's choice within 15 feet that can hear it must make a DC 15 Constitution saving throw, taking 14 (4d6) necrotic damage on a failed save, or half as much damage on a successful one. On a failed save, the creature is also stunned until the end of its next turn as nausea and visions of rot overwhelm it.
The Quincemort Host is a ritualic amalgam born where desperate cultists chained multiple corpses together in an attempt to bind loyalty beyond death. The rite, first recorded at the ruined temple of Vellog Altar, fused five ghastly cadavers with a single animating hunger. Over generations these altars have produced whole bands of Quincemort Hosts, each tied to the altar that birthed it and to the scent of the sacrificial oil used in the original rite. Adventuring parties and scholars alike fear the Host not only for its brutality but for its persistence: a Quincemort will drag itself back to its altar and ritually renew itself if given time and blood. The Order of the Iron Reliquary keeps a list of known altars, noting that the hosts are most active on moonlit nights and in the weeks following epidemics. Scholar Marek Ves argues that breaking a Host requires destroying the altar and the ritual bindings; local superstition says leaving even a single token from the original rite will call the Host back within a year.
Five emaciated torsos share a single braided sash of funerary cloth that runs through their ribcages like a corrupted umbilical; each torso has its own neck and head, skewed at slightly different angles, eyes milky or burned away, mouths stretched with blackened gums. Their limbs are uneven—some slightly elongated, one with a twisted forearm—covered in patchy, gray skin taut over bone, veins black as ink. Ragged ceremonial ties and rusted bits of altar iron hang from their hips. When moving the five figures tilt and slide in uncanny synchronization, producing small clouds of fetid vapor from rolled handfuls of sacrificial ash trapped in the sash. Scale: the assembled Host stands about as tall as a Large humanoid (roughly 8–9 feet wide across the five chests) and can spill through a standard 6.5-foot wide doorway, the five heads bobbing over a human's shoulder height.
