Ossifex Sentinel
Runecarved Skeleton
A lattice of bones etched with black runes, the Ossifex Sentinel moves with a chill precision, each step whisper-quiet despite its hulking frame. Where its hollow eyes should be, pools of cold, slow light gather and slip away as the runes along its ribs pulse. It does not hunt for flesh so much as for the spark of life—striking in patterns that bleed vitality rather than merely ruin flesh—leaving foes hollower and harder to mend once it passes.

Multiattack. The Ossifex Sentinel makes two Scythe attacks.
Scythe. Melee Weapon Attack: +6 to hit, reach 5 ft., one target. Hit: 10 (1d10+3) slashing damage plus 7 (2d6) necrotic damage. The target must succeed on a DC 13 Constitution saving throw or its hit point maximum is reduced by an amount equal to the necrotic damage taken. A creature whose hit point maximum is reduced to 0 by this effect dies; the reduction lasts until the target finishes a long rest and any removed hit points can't be regained until a lesser restoration or similar magic is cast.
Runic Drain (Recharge 5-6). The Ossifex unleashes the runes upon its chest in a 15-foot cone. Each creature in that area must make a DC 13 Constitution saving throw. On a failed save, a creature takes 21 (6d6) necrotic damage and the Ossifex regains hit points equal to half the necrotic damage dealt. On a successful save, the creature takes half as much damage and the Ossifex regains none.
Ossifex Sentinels are the products of a necromantic craft that blends funerary rite with sigil-magic. Millennia-old ossuaries in the Vale of Vorr contain dozens of sleepless specimens, each once the ossuary ward of a noble family or temple. Unlike banal skeletons raised by crude necromancers, Ossifex are created by aligning carved runes across bone joints; their creation is the purpose of few rites and fewer craftsmen. The Order of Keir, a cloister of rune-scribes, long used these sentinels to guard crypts and enforce tomb law, binding them to oath-runes that punished disturbance by leeching the intruder's vitality. Legends say Archivist Hesk of the Glass Spire cataloged the first true Ossifex three centuries ago and wrote of their appetite for life as a 'quiet rot' that undermines healing. In recent decades, rogue rune-scribes and battle-mages have adapted the process, constructing Ossifex that serve as front-line soldiers—animated not by servitude but by a perverse hunger embedded in their runes. Hunting them is dangerous because their attacks sap not only hit points but also the target's resilience; healers who attempt to restore victims without removing the runes sometimes find their spells failing or even feeding the sentinel's regeneration.
A humanoid skeleton standing roughly as tall as a human knight (about 6 to 6.5 feet), its bones are unnaturally pale with a faintly glossy sheen like old ivory waxed by centuries. Fine, black runes spiral and cross-cut along the collarbones, down the spine, ring the joints, and form a lattice across the breastplate of rib—each rune looks hand-carved and then filled with a tar-like pigment that seems to crawl when viewed peripherally. Where the skull's eyes would be, shallow hollows glow with slow, cold phosphorescence. The jaw hangs slightly open, and some specimens have thin bands of rusted metal wire across the wrists and ankles where ritual bindings were once secured. The overall texture is brittle and ancient, but joints move with unnerving smoothness; tiny bone dust flakes drift away with each step.
