Qil'ith Quartet
Many‑Minded Serpent
A writhing bundle of four serpentine bodies, each pair sharing a single, split skull that bears two cold, unblinking eyes and separate snapping jaws. The Qil'ith Quartet moves like a single animal when stalking prey, yet each head darts independently — tasting the air for different fears. Its hiss is a discordant chorus, and when it attacks two heads strike in coordinated salvos that leave bleeding punctures and a lingering, paralytic chill.

Multiattack. The Qil'ith Quartet makes two Bite attacks, choosing heads from any of its two-headed segments.
Bite. Melee Weapon Attack: +5 to hit, reach 5 ft., one target. Hit: 7 (1d6+3) piercing damage. If the target is a creature, it must succeed on a DC 12 Constitution saving throw or suffer partial numbness: the target has disadvantage on Dexterity checks and Dexterity saving throws until the end of its next turn.
Discordant Hiss (Recharge 5–6). The Quartet emits a sudden cacophony of hisses and strikes with blinding speed. Every creature of the Quartet's choice within 10 feet that can hear it must succeed on a DC 12 Wisdom saving throw or be frightened until the end of their next turn. A frightened creature that fails the save by 5 or more is also restrained by a wave of paralytic chill until the end of its next turn.
Scholars of the Marshwardens argue whether Qil'ith are aberrations or a rare offshoot of viperkind; most agree they are the result of arcane experimentation carried out by the forgotten cult of the Twofold Tongue. Excavations beneath the ruined shrine of Kexu-Thar uncovered stone bas-reliefs of twin skulls and a ritual chamber littered with coil marks and old bloodstains — the likely birthplace of many wild Qil'ith populations. In the wild, Qil'ith den in tangled reedbeds, collapsed culverts, and the root hollows of swamp oaks, preferring to ambush rather than pursue. Hunters of the river town Maren's Reach tell of single beasts that behave like packs, dividing attention between perimeter and prize with terrifying efficiency. The naturalist Isen Hatt has written a small, oft‑ignored pamphlet titled "On Many‑Minded Serpents," warning keepers to never approach two heads at once; she believes each pair of heads shares a quiet, empathic bond and will sacrifice one body to save its twin.
Four slender snake bodies (each about 6–7 feet long) coil together into a tangled mass no larger than a barrel. They are paired into two twin‑headed segments: each segment's skull is slightly flattened and split into two distinct crania fused at the temporal bones. Scales are dull green mottled with swamp brown, small and overlapping like fish scales. Each head bears two independently mobile jaws lined with needlelike teeth; thin, forked tongues flick constantly. Minor cartilage ridges run down the neck where the two skulls meet, and faint scarred lines (like healed ritual cuts) mark the dorsal scales. When uncoiling, the quartet forms a low, undulating mound; when striking, two heads lunge forward while the other two rear back, balancing the body.
