Oculorift

Reflected Menace

The Oculorift is a disquieting congregation of small, animate eyes that hang suspended within mirrors and other polished surfaces. Seen only as reflections, the swarm studies its prey with unnervingly coordinated intent, whispering without sound as its gaze probes for fears and secrets. When it strikes, it lashes out with thin, needle-like projections of light that pierce through a reflection into the real world — a strike that feels like being watched from beneath the skin.

Actions

Multiattack. The Oculorift makes two Gaze Needle attacks.

Gaze Needle. Ranged Weapon Attack: +4 to hit, range 30 ft., one target. Hit: 6 (1d6+3) piercing damage and 3 (1d6) psychic damage as an aching pressure of being observed.

Mirror Lure (Recharge 5-6). The swarm targets one creature that can see its own reflection in a mirror or polished surface within 30 ft. The target must succeed on a DC 11 Wisdom saving throw or be frightened until the end of its next turn and have disadvantage on Wisdom (Perception) checks that rely on sight.

Lore

Oculorifts are thinly understood by scholars; they are thought to be born where powerful images and longing concentrate — abandoned dressing rooms, sealed reliquaries with polished plaques, and the glass windows of forgotten manses. Folklore claims they are remnants of obsessions given shape, the unblinking consequence of years spent staring into a mirror, seeking a truth that will not come. The research notes of Archivist Veldun of the Glassward Monastery describe a field study where a single household mirror produced a steady stream of Oculorifts for nearly a month before the mirror cracked and the phenomenon ceased. They are solitary in motive but communal in action: small groups of eyes coordinate to exploit a single victim's weakness. Oculorifts prefer to feed on attention and psychic residue — the emotions and secrets reflected back at them — but they will physically injure those who attempt to destroy the mirrors that shelter them. Cults of reflection, such as the Observatory of Quiet Watching, sometimes attempt to bind Oculorifts as oracles; most attempts end with the cultists becoming husks of staring, their minds siphoned by the swarm.

Appearance

Render the Oculorift as a loose cluster of dozens of small eyes of varying species (human, feline, insectile) floating a few inches from a mirror's surface, their lids rimmed with jagged, hairlike filaments. Each eye's iris should be glossy and too-bright, with tiny, needlelike beams of light occasionally lancing from the pupil like fine threads. The cluster has no central body; instead, faint, smoky tendrils of reflected shadow connect the eyes, suggesting nervous tissue in reflection only. Scale: show the mirror propped against a wooden dresser and include a standing human silhouette in the reflection for size reference (the swarm should fit comfortably within a 2-foot-diameter area). Textures: high-contrast glossy pupils, scratched mirror glass, stippled skin around some eyes, and crosshatched shadow linking them.
