Oculisym
Symbiotic Watcher
Like a living iris slipped into a socket, the Oculisym appears at first as a miraculous prosthetic eye — glossy, faintly veined, unnervingly attentive. It rarely moves across the floor or ceiling; instead it waits in a socket, observing and occasionally twitching its pupil to track thoughts and secrets. What makes it terrifying is the intimacy of its parasitism: once grafted, it alters a host’s vision and perceptions in uncanny ways, sharing sights with no regard for consent and sometimes refusing to let the host look away.

Multiattack. The Oculisym makes two Bite attacks.
Bite. Melee Weapon Attack: +4 to hit, reach 5 ft., one target. Hit: 5 (1d4+2) piercing damage.
Graft. Melee Weapon Attack: +4 to hit, reach 5 ft., one willing or incapacitated humanoid's eye. Hit: 1 (1d1) piercing damage, and the Oculisym attaches itself to the creature's eye socket. While attached, the Oculisym cannot be forcibly removed except by a successful DC 12 Strength (Athletics) or Dexterity (Sleight of Hand) check made by the target or by someone else using an action. The Oculisym treats the host's senses as its own: the host gains darkvision 60 ft. (if it did not already have it) and a +2 bonus to Wisdom (Perception) checks that rely on sight. The Oculisym can use its movement while attached to peek from the socket (it gains full use of its listed speed for this purpose).
Blindness Siphon (Recharge 5-6). The Oculisym emits a sickly internal gleam targeted at the host eye. The creature wearing the Oculisym must succeed on a DC 12 Wisdom saving throw or be blinded for 1 minute. A blinded creature can repeat the saving throw at the end of each of its turns, ending the effect on a success. While the Oculisym is attached, it can use this ability once and then must recharge.
Oculisym are born of the deep hunger of aberrant intelligences that lick at the borders of sight. Tales speak of their emergence in hermetic workshops and plague-haunted infirmaries where surgeons and mages experimented with replacing lost eyes. Instead of dying, some of those eyes—threaded with a wrong kind of awareness—learned to graft and whisper. The Guild of Orbweavers, a small cabal of ocular artisans in the port city of Verran Hold, keeps a stunted record of complaints and contracts involving Oculisym, and warns apprentices to seal all discarded ocular tissue. Scholars differ on the Oculisym's motive. Archivist Voren of the University of Kest claims they are collectors, driven to accumulate visual impressions to build an internal atlas; others, like the field-worker Sister Greth of the Pestilence Ward, argue they are predators of attention, fattened on private moments. Oculisym are most often encountered already in place—sold to one-eyed sailors, sewn into the sockets of injured scouts, or smuggled into noble households as ‘miracle’ prosthetics. A few secretive cults, such as the Lanternless Circle, covet them as living reliquaries of forbidden visions.
A single, palm-sized eye roughly 1.5–2 inches across; the sclera is a faintly translucent alabaster threaded with thin, almost metallic veins that shimmer when exposed to light. The iris is a milky, opaline disc flecked with iridescent lodestars; the pupil is a vertical slit that can widen and probe like a cat’s. Beneath the visible surface, faint concentric rings pulse as if miniature muscles spool; a fringe of soft, membranous filaments (each the width of a sewing thread) ring the rear and can coil like tendrils to anchor the eye in flesh. When detached, it moves with a sluggish, deliberate crawl on its filaments, leaving a moist, ink-black residue. Scale reference: about the size of a large coin placed next to a human eye, with the filaments long enough to tuck into an adult human eye socket.
